- Insurance information
- Referral letter from your GP or healthcare professional ( if required )
- List of medications that you might be on
- Any copies of previous investigations & reports,such as X Rays & MRI. These may be on a CD .
If you need to cancel an appointment
Please contact the office during business hours and allow at least 24 hours’ notice so that we can offer your appointment time to patients on our waiting list.
